Our main office hours are 9:00 a.m. to 5:00 p.m. Monday through Friday. Evening and Saturday hours are available at certain locations. After hours emergency care is available by calling 402-572-2020 or 402-559-2829. The answering service will notify the doctor on call.
Appointments
Appointments can be scheduled by calling our Lakeside office at 402-934-7700, 72nd Street office at 402-572-2020 or 800-775-5909, Council Bluffs appointments at 712-325-4999. If you cannot keep an appointment, please call us in advance to let us know and we’ll be happy to reschedule it.
New Patients
Before your first appointment you will be asked to fill out a history form which asks for your personal, medical and insurance information. Bring your insurance and Medicare cards with you so we can make a copy for our records.
Office Visit
We want to give you the most thorough eye examination possible. Please help by bringing the following:
- Current eye medications
- Names and dosages of all medications you are taking
- Names of any drug(s) to which you have an allergy
- All eyeglasses that you presently wear, including prescription sunglasses
- Family medical history
- Your insurance and Medicare card(s)
- Previous contact lens prescriptions or contact lens boxes.
